What makes a post or reply require approval?

I posted 2 replies in SQL today, one needed approval and one didn't. The one that did, I think it may just be because it was very big.

Wondering what the actual rules are for when a post or reply does or does not require approval?

The rules are fluid .. I try to keep an eye out for post. Your post go thru in checked. If you are replying to
someone with a low post count you may be caught up in his/her approval process.
