COBOL/400 Printer report

Hi Guys,

I wrote a printer pgm but the thing is the report i wanted to be in the below format:

Personal order by Employee Number

Name Address1 EMPLOYEE NUMBER DATE OF BIRTH ----

XXXX 45466464 1404 JAN 13

XXXX 799797797 12212 FEB 7

XXXX 97979797 7979 MAR 13

This way it should be printed, I am posting my code and dds

It is zipped as Zippy, pls help me..
